These forms are a vital part of the coordination of your care.  Some questions might seem out of date or unrelated to your current condition or concerns, but please complete them fully anyway.  Understanding your past history of stressors and your body’s ability to deal with them will go a great way towards helping us give you more effective strategies going forward from this point.  Some of your history will be covered during your consultation, other aspects will be read over prior to your second visit where your Network Care recommendations will be presented.
